When Desmopressin's status or evidence changes, it's in the next morning's email.

What it is

Desmopressin is a lab-made version of vasopressin, the hormone the pituitary releases to tell the kidneys to hold on to water. Two small changes to the molecule make it last longer and remove most of vasopressin's effect on blood pressure. It works on the V2 receptor in the kidney, so less urine is made, and it also releases clotting factor VIII and von Willebrand factor from blood vessel walls.

It has been an FDA-approved drug since 1978. DDAVP tablets and injection treat central diabetes insipidus, the tablets treat bedwetting in children 6 and older, and the injection and Stimate nasal spray (which Drugs@FDA lists as discontinued, though Ferring updated its label in July 2026) treat mild hemophilia A and type 1 von Willebrand disease before surgery or after an injury. Two low-dose products were approved for nocturia caused by making too much urine at night: Noctiva nasal spray in 2017 and Nocdurna tablets under the tongue in 2018. In 3-month trials they cut night-time trips to the toilet by about 0.2 to 0.4 more per night than placebo. FDA's database now lists both as discontinued.

The main danger is low blood sodium. Desmopressin makes the body keep water, and if a person keeps drinking, sodium gets diluted. The injection and nocturia labels carry a boxed warning: severe hyponatremia can cause seizures, coma and death. In an insurance-claims study of 3,137 adults, mean age 70, low sodium was 13 times more common on desmopressin than on oxybutynin. WADA lists desmopressin as a masking agent, banned at all times.

Origin
Synthetic analog of the pituitary hormone arginine vasopressin. The first residue is deaminated (3-mercaptopropionic acid) and the arginine at position 8 is the D form, per the label's chemical name, 1-(3-mercaptopropionic acid)-8-D-arginine vasopressin.

How Desmopressin works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target Vasopressin V2 receptor (agonist)

Desmopressin binds vasopressin V2 receptors in the kidney's collecting ducts, which makes the kidney pull more water back into the blood and produce less, more concentrated urine. Through a separate effect it raises blood levels of factor VIII and von Willebrand factor within 30 minutes, which helps blood clot in people with mild hemophilia A or type 1 von Willebrand disease. The structural changes from vasopressin give it a longer action and much less effect on blood pressure.

  1. 1 Dose Given as a tablet, a tablet under the tongue, a nasal spray or an injection. Oral bioavailability is very low: per the DDAVP Tablets label, a 0.2 mg tablet works about like a 0.01 mg nasal dose.
  2. 2 V2 receptor binding Desmopressin binds V2 receptors on the collecting-duct cells of the kidney.
  3. 3 Water kept The kidney reabsorbs more water, so urine volume falls and urine becomes more concentrated. Tablet effects start in about 1 hour and peak at 4 to 7 hours.
  4. 4 Clotting factors released An IV dose of 0.3 mcg/kg raised factor VIII 2 to 3 fold in mild hemophilia in the first study. The rise starts within 30 minutes, peaks at 90 minutes to 2 hours, and the benefit lasts about as long as factor VIII does, 8 to 12 hours.
  5. 5 Risk if fluid continues If the person keeps drinking while the kidney holds water, blood sodium is diluted. That is the cause of the boxed hyponatremia warning.

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

A Evidence

Desmopressin is a long-established drug backed by randomized trials for each labeled use. For nocturia the benefit is real but small: about 0.2 to 0.4 fewer night-time voids than placebo over 3 months. For bedwetting a 2025 Cochrane review found children on desmopressin were about 3 times as likely as on placebo to reach 14 dry nights in a row. In mild hemophilia A and von Willebrand disease about 71% of patients reach a useful factor level, and the drug's main harm, low blood sodium, is well documented.

What nobody has measured yet

These are questions the published research has not answered.

  1. 01 The real-world rate of severe hyponatremia with the low-dose nocturia products outside trials; the 2019 claims study covered only the older formulations.
  2. 02 Whether the small trial benefit in nocturia (about 0.2 to 0.4 fewer voids a night) translates into fewer falls or better health in older adults.
  3. 03 Long-term safety of nightly use for years in people over 65, who had the most low-sodium events in trials.
  4. 04 Why about 3 in 10 people with mild bleeding disorders do not get an adequate factor response.

How long it stays in the body

Half-life
About 2.8 hours
About 2.8 hours after IV or nasal dosing; 1.5 to 2.5 hours after oral tablets. Rises to about 8.7 hours in severe kidney impairment.
Routes studied
Oral tablet, Sublingual tablet (Nocdurna), Intranasal spray, Intravenous or subcutaneous injection
Bioavailability
Oral tablets reach about 0.16% of IV exposure and about 5% of intranasal exposure, per the DDAVP Tablets label. Peak levels come 0.9 to 1.5 hours after oral or intranasal doses.
Notes
Cleared mainly by the kidneys. In 24 people given 2 mcg IV, half-life was 2.8 hours with normal kidneys and 4, 6.6 and 8.7 hours with mild, moderate and severe impairment, which is why the labels bar use below a creatinine clearance of 50 mL/min. Repeat doses for bleeding more often than every 48 hours give a weaker response (tachyphylaxis).

Groups with specific risks
  • Low blood sodium now or in the past
  • Excessive fluid intake or polydipsia
  • Use of loop diuretics or systemic or inhaled glucocorticoids
  • Kidney function below a creatinine clearance or eGFR of 50 mL/min
  • SIADH, or illness causing fluid or electrolyte imbalance (vomiting, diarrhea, fever)
  • Heart failure or uncontrolled high blood pressure (nocturia products)
  • Allergy to desmopressin
Interactions
  • Loop diuretics and systemic or inhaled glucocorticoids: contraindicated because they raise the risk of severe low sodium.
  • Drugs that also lower sodium (SSRIs, tricyclic antidepressants, chlorpromazine, opioids, thiazide diuretics, NSAIDs, lamotrigine, carbamazepine, chlorpropamide): check sodium more often.
  • Other vasoconstrictors: may add to the blood pressure rise seen with injected desmopressin.

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"Desmopressin will stop me waking up at night to pee."MixedSome studies agree, some do not

It only helps when the kidneys make too much urine at night (nocturnal polyuria). In the Nocdurna and Noctiva trials, people went from about 3 trips a night to about 1.5 to 2, but placebo groups also improved, so the drug's own effect was about 0.2 to 0.4 fewer trips a night.

The claim"Low-dose desmopressin is too weak to cause low sodium."Not supportedThe evidence does not back this

Even the low-dose nocturia products carry the boxed warning. In the Nocdurna trials about 4% of men 65 and older on 55.3 mcg had sodium of 129 mmol/L or lower, and 2 men on that dose had serious hyponatremia.

The claim"You can drink normally after taking it."Not supportedThe evidence does not back this

The labels tell patients to limit fluids from 1 hour before a dose until 8 hours after (24 hours for Stimate). Drinking while the drug is working is what dilutes the blood sodium.

The claim"DDAVP treats any bleeding disorder."Not supportedThe evidence does not back this

It is labeled only for hemophilia A and type 1 von Willebrand disease with factor VIII above 5%. It is not for severe von Willebrand disease, and in type 2B it can cause clots. In surgery patients without a bleeding disorder, a Cochrane review found it did not reduce transfusions.

Frequently asked questions
Is desmopressin a peptide?
Yes. It is a 9-unit peptide made by changing two parts of vasopressin, the body's antidiuretic hormone. The changes make it last longer and take away most of its effect on blood pressure.
Why do I have to limit water on desmopressin?
Desmopressin makes the kidneys keep water. If you keep drinking, the extra water dilutes the sodium in your blood, which can cause headache, confusion, seizures and in severe cases death. Labels say to keep fluids to a minimum from 1 hour before a dose until 8 hours after.
Are Nocdurna and Noctiva still available?
FDA's Drugs@FDA database lists both as discontinued. Generic desmopressin tablets, nasal sprays and injections are still sold for their labeled uses.
How does desmopressin help with bleeding?
It releases stored factor VIII and von Willebrand factor from blood vessel walls. In the first study, a 0.3 mcg/kg dose raised factor VIII 2 to 3 fold in people with mild or moderate hemophilia A, and 8 patients with mild hemophilia or von Willebrand disease then had dental or major surgery without plasma concentrates. Two of the first 4 dental patients still needed concentrates.
Is desmopressin banned in sport?
Yes. WADA lists desmopressin under S5 as a masking agent, prohibited in and out of competition. Athletes who need it for a medical condition must apply for a Therapeutic Use Exemption.
Who should not take desmopressin?
People with low sodium now or in the past, people who drink large amounts of fluid, those on loop diuretics or steroid pills or inhalers, and people with poor kidney function (creatinine clearance below 50 mL/min). Adults 65 and older need more frequent sodium checks.
