Which peptides cause weight loss, and how much people actually lost

If you are weighing semaglutide, tirzepatide, retatrutide or a peptide sold online as a fat burner, the line is clear. The GLP-1 family has trials in thousands of people. Nearly everything else has mouse data or a failed trial. This page puts the numbers side by side.

Compare the trial results
GLP-1 drugs have far stronger evidence than anything else here. Semaglutide and tirzepatide have randomized trials in tens of thousands of people. In their main obesity trials people lost about 15% and 21% of body weight on average, against 2 to 3% on placebo.
More receptors has meant more weight lost so far. Retatrutide acts on three hormone receptors and averaged 25.0% loss at 80 weeks in TRIUMPH-1. It is not approved; Lilly says it will file with the FDA in early 2027.
Most of the weight comes back after stopping. People switched from tirzepatide to placebo in SURMOUNT-4 regained 14% of their body weight within a year. People who stayed on the drug kept losing.
Part of the weight lost is lean mass. DEXA scans in the semaglutide and tirzepatide trials put lean mass at roughly a quarter to two fifths of the total weight lost.
The online fat-burner peptides have no human proof. AOD-9604 and HGH fragment 176-191 failed a 536-person trial in 2007. MOTS-c, 5-Amino-1MQ and SLU-PP-332 have only been tested in mice.
Compounded copies are not the trial drugs. Every trial on this page used the approved branded product. FDA ended the shortage exemption for compounded copies of semaglutide and tirzepatide in 2025.

Retatrutide in TRIUMPH-1

2,339 adults with obesity or overweight and no diabetes, 80 weeks. Highest dose arm: 25.0% lost on average, against 3.9% lost on placebo. Placebo-adjusted difference: 21.1 points.

In clinical trials

Trials ran for different lengths (48 to 80 weeks) and enrolled different people, so bars are not a head-to-head ranking. Only two trials compared drugs directly: tirzepatide beat semaglutide 20.2% to 13.7% (SURMOUNT-5), and tirzepatide beat CagriSema 25.5% to 23.0% (REDEFINE 4). Tap a bar for the trial details.

AOD-9604 and HGH fragment 176-191: no significant loss

The 24-week OPTIONS trial enrolled 536 adults with obesity and randomized 502 of them. Weight loss against placebo was too small to reach statistical significance, the company ended the program in February 2007, and the full results never appeared in a journal. FDA briefing document

GLP-1, dual and triple agonists: which receptors each one hits

These drugs copy gut hormones that tell the brain you have eaten. Each new class adds a receptor. Pick a class to see the receptors it acts on, the organs those receptors reach, and its best trial result.

  • GLP-1 receptorCuts hunger and makes food less rewarding in the brain, slows how fast the stomach empties, and makes the pancreas release insulin only when blood sugar is high.
  • GIP receptorAdds insulin release after meals and acts on fat tissue. Animal work suggests it may also blunt GLP-1 nausea; that part is not proven in people.
  • Glucagon receptorPushes the liver to burn more fat and raises energy use. On its own it would raise blood sugar; the GLP-1 half of the drug offsets that.

The largest average weight loss of any drug in a published phase 3 trial. Not approved; no heart-outcome data yet.

Stopping the drug, and what the weight lost was made of

Two results that change how to read every number above. Both come from the same randomized trials.

What happened after switching to placebo

Change in body weight over the following year, from the day of the switch

Tirzepatide, SURMOUNT-4 670 adults who first lost 20.9% over 36 weeks
Stayed on drug -5.5%
Switched to placebo +14%
Source: SURMOUNT-4, 52 weeks
Semaglutide, STEP 4 803 adults who first lost 10.6% over 20 weeks
Stayed on drug -7.9%
Switched to placebo +6.9%
Source: STEP 4, 48 weeks

Bars grow left for further loss and right for regain. Center line is the weight on the day of the switch.

What the weight lost was made of

Share of total weight lost that was fat versus lean mass, measured by DEXA scan in a subgroup of each trial

Semaglutide, STEP 1
Fat about 61% Lean 39%
Source: Wilding 2021, DEXA substudy
Tirzepatide, SURMOUNT-1
Fat about 75% Lean 25%
Source: Jastreboff 2022, DEXA substudy

Lean mass on a DEXA scan includes muscle, organs and water, so it is not the same as muscle alone. Losing some lean mass happens with any large weight loss, including from dieting. The trials measured it by scan; they did not test whether it changed strength.

What the muscle peptides actually showed

The GLP-1 and amylin drugs compared

Main obesity trial in adults without diabetes for each drug. US status comes from the database and changes as approvals land.

Drug Receptors Main trial Average loss Form US status
Semaglutide GLP-1 STEP 1, 68 weeks 14.9% vs 2.4% placebo Weekly injection or daily tablet FDA-approved
Tirzepatide GIP + GLP-1 SURMOUNT-1, 72 weeks 20.9% vs 3.1% placebo Weekly injection FDA-approved
Retatrutide GIP + GLP-1 + glucagon TRIUMPH-1, 80 weeks 25.0% vs 3.9% placebo Weekly injection In clinical trials

Mazdutide GLP-1 + glucagon GLORY-2, 60 weeks 16.7% vs 1.5% placebo Weekly injection Approved outside US
Survodutide GLP-1 + glucagon SYNCHRONIZE-1, 76 weeks 13.0% vs 5.4% placebo Weekly injection In clinical trials
Eloralintide Amylin Phase 2, 48 weeks 20.0% vs 0.4% placebo, on-drug analysis Weekly injection In clinical trials
Orforglipron GLP-1 (not a peptide) ATTAIN-1, 72 weeks 12.4% vs 0.9% placebo Daily pill FDA-approved

"On-drug analysis" counts only people who kept taking the drug, which makes the number look larger than an analysis of everyone randomized. Orforglipron is a small-molecule pill, not a peptide, and has no profile in the peptide database.

Survodutide and retatrutide have never been tested in the same trial. Their separate weight, liver and side-effect results are compared in survodutide vs retatrutide.

Mazdutide and tirzepatide have not been tested in the same trial either. Their results in Chinese adults are set side by side in mazdutide vs tirzepatide.

The research compounds, and what was actually shown

These are the fat-loss compounds sold without a prescription as research chemicals. None is approved for anything. Here is the best evidence for each.

What the evidence does not show

Questions that come up constantly and that no published trial has answered.

  1. 01
    Decades of use

The longest placebo-controlled weight data run about 3 years (the SURMOUNT-1 extension, 176 weeks). Nobody has data on 20 years of continuous use. Source

  2. 02
    Retatrutide heart outcomes

No trial has reported heart attacks, strokes or deaths on retatrutide, and no trial has run past about 2 years. Source

  3. 03
    Compounded and research-grade GLP-1s

No trial tested compounded semaglutide salt forms or vials sold "for research use only". FDA says salt forms such as semaglutide sodium are not the same active ingredient as the approved drug. Source

  4. 04
    Stacks with research peptides

No published trial has combined a GLP-1 drug with BPC-157, MOTS-c, AOD-9604 or any other research peptide.

  5. 05
    Protecting muscle

No compound on this page has been shown in a published trial to protect muscle during GLP-1 weight loss. The trials measured lean mass by DEXA, not strength. Source

  6. 06
    Fat burners in people

Not one of the research compounds sold for fat loss has a positive, published human trial. The best result for any of them is in mice. Source

Frequently asked questions
Which peptide causes the most weight loss?
In published phase 3 trials, retatrutide has the largest average loss: 25.0% at 80 weeks in TRIUMPH-1, against 3.9% on placebo. It is investigational and not approved anywhere. Among approved drugs, tirzepatide (Zepbound) has the largest average loss, 20.9% at 72 weeks in SURMOUNT-1.
Is tirzepatide better than semaglutide for weight loss?
In the one head-to-head obesity trial, SURMOUNT-5, people on tirzepatide lost 20.2% of body weight on average at 72 weeks, against 13.7% on semaglutide. Waist size fell 18.4 cm versus 13.0 cm. Semaglutide has a longer safety record and the only finished heart-outcomes trial in people without diabetes (SELECT).
Is retatrutide approved?
No. Retatrutide is in phase 3 trials and not approved by the FDA or any other regulator as of October 2026. Lilly says it plans to file with the FDA in the first quarter of 2027. Vials sold online as retatrutide are unapproved drugs, and FDA states it cannot be compounded.
Do AOD-9604 or HGH fragment 176-191 work for fat loss?
Not in people. They share the same sequence, and the company that developed it ran six placebo-controlled trials in about 900 adults. The decisive 24-week trial found no statistically significant weight loss compared with placebo, and the program was ended in 2007. The fat-burning results come from mice and rats.
Will I regain the weight if I stop a GLP-1 drug?
Most people in the trials did. In SURMOUNT-4, people who had lost about 21% on tirzepatide and were switched to placebo regained 14% of their body weight over the next year. In STEP 4, people switched off semaglutide regained 6.9% over 48 weeks while those who stayed on lost a further 7.9%.
Are compounded semaglutide and tirzepatide the same as Wegovy or Zepbound?
They are not the products tested in the trials. The shortages ended in 2024 and 2025, and FDA ended the exemption that had let pharmacies make copies. FDA has warned that some compounders used salt forms, such as semaglutide sodium, that are not the same active ingredient as the approved drug.
Are these drugs banned in sport?
Semaglutide and tirzepatide are not on the WADA Prohibited List; WADA monitors them from 2026. Unapproved drugs such as retatrutide and cagrilintide fall under S0, non-approved substances, which is banned at all times. AOD-9604 and HGH fragment 176-191 are named under S2, and MOTS-c is named under S4.
