What it is

IGF-1 DES, written des(1-3)IGF-I in research papers, is insulin-like growth factor 1 missing its first three amino acids, glycine, proline and glutamic acid. That leaves 67 of IGF-1's 70 amino acids. It occurs naturally in small amounts: it has been isolated from cow colostrum, human brain and pig uterus, probably cut from IGF-1 after it is made. Losing the glutamic acid at position 3 means the IGF binding proteins that normally hold most IGF-1 in the blood barely grip it, so more of it is free to reach the IGF-1 receptor.

In cultured cells it is generally about 10 times more potent than IGF-1. In rats it was 2.5 to 3 times more potent at restoring growth in diabetic rats, improved nitrogen balance after gut and kidney surgery, and had its strongest effects on the gut. It also cleared from rat blood almost 4 times faster than IGF-1. It is the same idea as IGF-1 LR3, which reaches the same result with an arginine swap and an extra 13-amino-acid tail instead of a deletion.

No human trial, pharmacokinetic study or safety study of IGF-1 DES has been published, and it was never developed as a drug. What is known about IGF-1 in humans comes from mecasermin (Increlex), recombinant native IGF-1, whose label warns of low blood sugar severe enough to cause seizures, raised pressure in the skull, tonsil and adenoid swelling, and cases of cancer in treated children. WADA bans IGF-1 and its analogues, including IGF-1 DES, at all times, and anti-doping labs test for it.

How IGF-1 DES works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target Type 1 IGF receptor (IGF-1R) agonist with weak IGF binding protein affinity

IGF-1 DES activates the same type 1 IGF receptor as native IGF-1. Its extra potency comes from barely binding IGF binding proteins, the absence of glutamic acid at position 3 being the key change, so more of it is free to reach tissues. In rats it stayed largely unbound in the blood, spread into a larger volume, reached the gut, skin and brain more than IGF-1, and was cleared about 4 times faster.

  1. 1 Escapes binding proteins Without Gly-Pro-Glu at the front, the peptide binds IGF binding proteins much less, so it does not get parked in the 150 kDa complexes that hold most IGF-1 in blood.
  2. 2 Reaches tissues In rats, more labeled IGF-1 DES than IGF-1 ended up in adrenals, brain, skin, stomach and intestine.
  3. 3 Activates IGF-1 receptor It binds IGF-1R on muscle, gut and other cells and switches on growth and protein synthesis signaling.
  4. 4 Growth effects in animals In rats it raised muscle protein synthesis, slowed muscle protein breakdown and grew gut tissue more than native IGF-1.
  5. 5 Rapid clearance Plasma clearance in rats was 4.59 mL/min/kg vs 1.20 for IGF-1, so it acts fast and is gone fast.

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

D Evidence

There are no published human studies of IGF-1 DES: no trials, no pharmacokinetics and no safety data. Rat studies show it is more potent than IGF-1 at restoring growth and preserving muscle protein, with the strongest effects on the gut, but it also clears from blood faster. A 1996 review by the group that studied it most said clinical uses had not yet been evaluated, and none have been since.

Notes
Rat plasma clearance was 4.59 mL/min/kg vs 1.20 for IGF-I, and steady-state volume of distribution 461 vs 167 mL/kg, because it is not held by binding proteins. After a single intramuscular dose in rats, the intact peptide was detectable for 24 hours with a sensitive anti-doping assay. Rat serum contains an acid protease that cuts intact IGF-I into des(1-3)IGF-I, but only the free IGF-I fraction, not IGF-I held in binding-protein complexes. For comparison, the mecasermin label gives a half-life of about 5.8 hours for native IGF-1 in children.

No human half-life has been published for IGF-1 DES, so we do not draw a clearance curve. Half-lives quoted online for it are usually estimates from animal data.

  • Pregnancy and breastfeeding: no data
  • Children and teens: never studied
  • Competitive athletes under WADA rules
Interactions
  • Insulin and other glucose-lowering drugs: additive hypoglycemia risk expected from IGF-1 receptor signaling; not studied for IGF-1 DES.

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"IGF-1 DES is 10 times stronger than IGF-1 in the body."Not supportedThe evidence does not back this

The roughly 10-fold figure comes from cultured cells. In rats it was 2.5 to 3 times more potent at restoring growth, and no human data exist.

The claim"IGF-1 DES works only on the muscle you inject it into."UntestedNobody has measured this in people

No human study has looked. In rats it spread into a larger volume than IGF-1 and reached gut, skin and brain, with its strongest effects on the gut.

The claim"IGF-1 DES is natural, so it is safe."Not supportedThe evidence does not back this

Small amounts occur naturally, but injected doses have never been tested for safety in people. Native IGF-1 carries label warnings for hypoglycemic seizures and malignancy.

The claim"IGF-1 DES builds muscle in adults."UntestedNobody has measured this in people

Rat studies show better nitrogen balance and less muscle protein breakdown, mostly in sick or surgically injured rats. No human study exists, and a year of native IGF-1 in older women did not add lean mass.

Frequently asked questions
What is the difference between IGF-1 DES and IGF-1 LR3?
Both are engineered or modified versions of IGF-1 that binding proteins barely hold. IGF-1 DES is shorter: the first three amino acids are removed. IGF-1 LR3 is longer: an arginine replaces glutamic acid at position 3 and a 13-amino-acid tail is added. In diabetic rats both were 2.5 to 3 times more potent than IGF-1. Neither has been tested in humans.
Is IGF-1 DES FDA approved?
No. It was never developed as a drug and has no human trials. The only approved IGF-1 drug in the US is mecasermin (Increlex), native IGF-1 for children with severe IGF-1 deficiency.
How long does IGF-1 DES last?
Nobody has measured it in humans. In rats it was cleared from blood about 4 times faster than native IGF-1 because binding proteins do not hold it.
Is IGF-1 DES detectable in drug tests?
Yes, on anti-doping tests. WADA bans IGF-1 and its analogues at all times, and labs have published mass spectrometry methods that detect Des(1-3)-IGF-I in blood.
Does IGF-1 DES cause low blood sugar?
It has not been measured in people. Low blood sugar affected 42% of children in trials of the approved native IGF-1 drug, so a similar risk is plausible, but it is unmeasured for IGF-1 DES.
