When Pemvidutide's status or evidence changes, it's in the next morning's email.
What it is
Pemvidutide is a once-weekly injectable peptide from Altimmune that activates two receptors in equal measure: the GLP-1 receptor, which cuts appetite, and the glucagon receptor, which acts directly on the liver to burn fat. It carries a built-in albumin-binding section (Altimmune calls it the EuPort domain) that stretches it to weekly dosing and slows how fast it reaches the blood.
In the 48-week MOMENTUM obesity trial, which Altimmune has reported but not published in a journal, 391 adults without diabetes lost 10.3%, 11.2% and 15.6% of body weight on 1.2, 1.8 and 2.4 mg, against 2.2% on placebo. Altimmune has since focused on the liver. In the IMPACT trial of 212 people with biopsy-proven MASH and F2 to F3 fibrosis, MASH cleared without fibrosis getting worse in 58% (1.2 mg) and 52% (1.8 mg) at 24 weeks versus 20% on placebo, but the fibrosis-improvement goal was missed.

Altimmune reported that the FDA gave pemvidutide Breakthrough Therapy designation for MASH in January 2026, and the company's PERFORMA phase 3 trial (about 1,800 people) began in August 2026, with 52-week biopsy results expected in 2029. In July 2026 the company reported that a 100-person alcohol use disorder trial cut heavy drinking days more than placebo; those results are not yet peer reviewed. It is not approved anywhere.

Origin
Synthetic peptide-based GLP-1/glucagon dual receptor agonist with balanced 1:1 activity, built with Altimmune's proprietary EuPort domain for albumin binding. The full structure has not been published in a source we could verify.
How Pemvidutide works
What it binds to, and what happens inside the body after that, step by step.
TargetGLP-1 receptor and glucagon receptor (balanced dual agonist)
Pemvidutide activates GLP-1 and glucagon receptors at a 1:1 ratio. GLP-1 activity lowers appetite and food intake. Glucagon activity acts on the liver, pushing it to burn fat and make less new fat, which is why liver fat falls faster than weight alone would explain. GLP-1 signaling also touches the brain's reward and craving pathways, the reason it is being tested in alcohol use disorder.
1Albumin bindingThe EuPort domain binds albumin in the blood, slowing release so one shot lasts a week.
2GLP-1 receptorAppetite and meal size fall; weight dropped 15.6% at 48 weeks on 2.4 mg in MOMENTUM.
3Glucagon receptor in the liverThe liver burns more fat and makes less; liver fat fell 68.5% in 12 weeks on 1.8 mg in a 94-person trial.
4Liver inflammation settlesIn IMPACT, MASH cleared without fibrosis worsening in 52% to 58% vs 20% on placebo at 24 weeks.
What the studies show
Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.
BEvidence
Several placebo-controlled human trials, including a peer-reviewed phase 2b MASH biopsy trial in the Lancet, show lower liver fat, MASH resolution and weight loss. It missed the fibrosis-improvement endpoint at 24 weeks. The 48-week obesity trial and the alcohol use disorder trial are reported mainly through company releases, and no phase 3 trial has reported.

What nobody has measured yet
These are questions the published research has not answered.
01Whether it improves fibrosis on biopsy; IMPACT missed that endpoint at 24 weeks and PERFORMA reads out in 2029.
02Whether it prevents cirrhosis, liver failure or death.
03Human half-life and full molecular structure.
04Long-term safety beyond 48 weeks.
05Whether an obesity phase 3 will ever run.
06Head-to-head comparisons with semaglutide, tirzepatide or survodutide.

Approval or regulatory milestoneResearch milestone
Legal and regulatory status
In clinical trials Being tested in human trials. Not approved anywhere yet.
No product containing Pemvidutide is approved as a medicine by the FDA.
Phase 3 in MASH (PERFORMA, started August 2026; 52-week readout expected 2029; final approval tied to liver events at about 60 months). FDA Fast Track and Breakthrough Therapy designations for MASH (Breakthrough announced January 5, 2026) and Fast Track for alcohol use disorder. Phase 2 in alcohol-associated liver disease (RESTORE). Altimmune's 2026 releases do not list an obesity phase 3. WADA: not named on the 2026 Prohibited List, but with no approval anywhere it falls under S0 (non-approved substances), prohibited at all times. This is our reading of the S0 rule, not a WADA ruling on pemvidutide by name.
Sport (WADA)
Prohibited. Tested athletes can fail a drug test for it.
Compounding pharmacies
Not eligible. Pemvidutide is not a component of any FDA-approved drug and does not appear on FDA's 503A bulk substance category lists (updated May 14, 2026). Products sold online as pemvidutide are unapproved research chemicals.
When the FDA or WADA changes Pemvidutide's status, it's in the next morning's email.
How long it stays in the body
Half-life
Not established
Long enough for once-weekly dosing; a human half-life value was not in a source we could verify
Routes studied
Subcutaneous injection (once weekly)
Bioavailability
Not published for humans in a source we could verify.
Notes
Altimmune says the EuPort domain binds albumin, extends half-life and slows entry into the blood, which may help tolerability. IMPACT gave 1.2 mg and 1.8 mg without dose titration; PERFORMA uses monthly steps up to 1.8 or 2.4 mg.
No human half-life has been published for Pemvidutide, so we do not draw a clearance curve. Half-lives quoted online for it are usually estimates from animal data.

Frequently asked questions
Is pemvidutide FDA approved?
No. It is in the PERFORMA phase 3 trial for MASH, which started in August 2026; 52-week results are expected in 2029.
How much weight do people lose on pemvidutide?
In the 48-week MOMENTUM trial, Altimmune reported 10.3% to 15.6% depending on dose, versus 2.2% on placebo. In the MASH trial, at lower doses, the company reported weight loss of 4.5% to 7.5% at 48 weeks. Neither result has been published in a journal.
What is pemvidutide used for?
It is being developed for MASH (fatty liver disease with inflammation and scarring), alcohol use disorder and alcohol-associated liver disease. It was also tested for obesity.
How is pemvidutide different from survodutide?
Both are weekly GLP-1/glucagon dual agonists aimed at obesity and MASH. Pemvidutide is from Altimmune and has 1:1 balanced activity; survodutide is from Boehringer Ingelheim and Zealand. They have not been compared in a trial.
Does pemvidutide help with alcohol cravings?
In the 100-person RECLAIM trial, 2.4 mg weekly cut heavy drinking days per week by 4.20 vs 2.75 on placebo over 24 weeks. These are company topline results, not yet peer reviewed.
