What it is

Pramlintide is a synthetic version of amylin, a hormone that beta cells in the pancreas release together with insulin after a meal. People with type 1 diabetes lack amylin as well as insulin. Pramlintide differs from human amylin at three positions, where it carries proline. It was sold as Symlin, injected before each main meal alongside insulin, and approved by FDA in 2005 for type 1 and type 2 diabetes.

The effect is modest. In a 52-week trial of 651 people with type 1 diabetes, HbA1c fell 0.29 to 0.34 points more than placebo, and weight fell 0.4 kg while the placebo group gained 0.8 kg. In 656 people with insulin-treated type 2 diabetes, 120 mcg twice daily cut HbA1c 0.62 points at 52 weeks and weight by 1.4 kg. The main risk is severe low blood sugar within 3 hours of the shot, which is why the label carries a boxed warning and tells people to cut mealtime insulin by half when starting.

Pramlintide matters today mostly as the first amylin drug. Its 48-minute half-life meant a shot before every main meal, and it never became a weight-loss drug. Newer long-acting amylin analogs such as cagrilintide, eloralintide and petrelintide are built on the same idea. Drugs@FDA lists every Symlin strength as discontinued, and drug references say it is no longer available in the United States.

How Pramlintide works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target Amylin receptor (agonist)

Amylin is stored with insulin in beta cells and released with it after eating. Pramlintide copies three of its effects: it slows stomach emptying, lowers the glucagon rise after meals so the liver releases less glucose, and acts on the brain to reduce food intake.

  1. 1 Injection before a meal Pramlintide peaks about 20 minutes after a subcutaneous injection and has a half-life of about 48 minutes, so it covers the meal it is given with.
  2. 2 Slower stomach emptying Food reaches the small intestine more slowly, without changing how much is absorbed overall, which flattens the post-meal glucose rise.
  3. 3 Lower glucagon after meals Pramlintide blunts the glucagon rise that insulin alone does not fix, so the liver puts out less glucose.
  4. 4 Fullness signal Acting through the brain, it increases fullness and lowers calorie intake, which explains the small weight loss in trials.

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

A Evidence

Pramlintide was an FDA-approved drug backed by several 6- to 12-month placebo-controlled trials in type 1 and type 2 diabetes. It lowered HbA1c by about 0.3 to 0.7 points and body weight by 1 to 2 kg compared with placebo. A 16-week phase 2 trial in obesity without insulin found about 3.7% placebo-adjusted weight loss, but it was never approved for weight management.

What nobody has measured yet

These are questions the published research has not answered.

  1. 01 Long-term outcomes such as heart events or diabetes complications: no outcome trial was run.
  2. 02 Use in children, pregnancy and breastfeeding, which were not established.
  3. 03 Whether a long-acting version would have mattered for weight; that question passed to newer analogs such as cagrilintide.
  4. 04 The exact date and reason for the US discontinuation, which we could not find in an official source.

How long it stays in the body

Half-life

Bioavailability
About 30% to 40% after subcutaneous injection.
Notes
Peak levels arrive about 20 minutes after injection. Because it clears within a few hours, it was injected immediately before each major meal, in a separate shot from insulin. Mixing it with insulin in one syringe changes its absorption, so the label forbids it.

Groups with specific risks
  • Hypoglycemia unawareness
  • Confirmed gastroparesis
  • Serious hypersensitivity to pramlintide or any product component
  • The label also says not to use it in people with HbA1c over 9%, recurrent severe low blood sugar in the past 6 months, poor adherence to insulin and glucose checks, drugs that speed gut movement, or in children
Interactions
  • Insulin: must be given as a separate injection, never mixed in the same syringe; mealtime insulin is cut by 50% when starting.
  • Drugs that alter gut movement, such as anticholinergics like atropine, and drugs that slow nutrient absorption, such as alpha-glucosidase inhibitors: the label says pramlintide should not be considered with them.
  • Oral drugs where speed matters, such as pain relievers, antibiotics and oral contraceptives: take at least 1 hour before or 2 hours after the injection.

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"Pramlintide is a weight-loss drug."Not supportedThe evidence does not back this

It was approved only as an add-on to mealtime insulin. Weight loss in the diabetes trials was 1 to 2 kg versus placebo, and the 16-week obesity trial (3.7% placebo-adjusted) never led to a weight indication.

The claim"Pramlintide replaces insulin."Not supportedThe evidence does not back this

It was only approved for people already using mealtime insulin and does not lower glucose enough on its own. Insulin doses are reduced, not stopped.

The claim"Amylin drugs are the next step after GLP-1 drugs."MixedSome studies agree, some do not

Long-acting amylin analogs such as cagrilintide are in late-stage trials, alone and with semaglutide. Pramlintide itself was too short-acting to be used that way.

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

Pramlintide + mealtime insulin

The approved use: replacing amylin alongside insulin in people who make little or none.

EvidenceShown in 52-week RCTs to lower HbA1c by about 0.3 to 0.6 points and weight by 1 to 2 kg, at the cost of more nausea and early severe lows.

Amylin + GLP-1 (pramlintide idea, modern form)

Amylin and GLP-1 cut appetite through different brain circuits, so combining them was designed to add weight loss.

EvidenceTested with cagrilintide plus semaglutide (CagriSema), not with pramlintide. See the cagrisema profile.

Frequently asked questions
Is Symlin still available?
Not in the US. Drugs@FDA lists every Symlin strength as discontinued, and Davis's Drug Guide states it is no longer available in the United States.
What does pramlintide do?
It copies amylin, a hormone released with insulin after meals. It slows stomach emptying, lowers the glucagon rise after eating and reduces appetite, which flattens blood sugar spikes after meals.
Why does Symlin cause low blood sugar?
It slows the arrival of food from the stomach, while mealtime insulin keeps working on its usual schedule. That mismatch can cause severe lows within 3 hours, so the label tells people to cut mealtime insulin by 50% when starting.
How is pramlintide different from cagrilintide?
Both are amylin analogs. Pramlintide lasts about 48 minutes and was injected before every meal. Cagrilintide is built to last about a week and is being tested for weight loss, alone and combined with semaglutide.
Is pramlintide banned in sport?
It is not named on the WADA Prohibited List. Because WADA's S0 class covers discontinued drugs that have no current approval, athletes should ask their anti-doping organization before using it.
