When Gonadorelin's status or evidence changes, it's in the next morning's email.

What it is

Gonadorelin is a lab-made copy of gonadotropin-releasing hormone (GnRH), the 10-amino-acid hormone the hypothalamus releases in pulses to tell the pituitary to make LH and FSH. LH and FSH in turn drive testosterone, estrogen, sperm and egg production. Gonadorelin is identical to the natural hormone, which is why it lasts only minutes in the blood: its initial half-life is about 2 to 10 minutes.

How it is given decides what it does. Pulses every 60 to 120 minutes switch the reproductive axis on; a constant supply switches it off, which is the principle behind long-acting GnRH drugs such as leuprolide. In the US it was approved as Factrel in 1982, a single 100 mcg injection used to test whether the pituitary can release LH, and as Lutrepulse in 1989, a pump that gives a small dose every 90 minutes to make women with hypothalamic amenorrhea ovulate. In trials of 268 women, 94% with primary hypothalamic amenorrhea ovulated. Both US human products are now discontinued. Outside the US, Lutrepulse has a Canadian monograph (revised 2016), France supplies it as Lutrelef under a shortage notice, and a UK product licence for a 100 mcg diagnostic injection was last updated in 2022.

Gonadorelin remains an FDA-approved veterinary drug: Factrel, Cystorelin, Fertagyl, OvaCyst and GONAbreed are used in cattle for ovarian cysts and to time insemination. In 2026 some men's health clinics prescribe compounded gonadorelin alongside testosterone, hoping to keep the testes working as hCG would. No published trial has tested that use. Pharmacies compound it on the basis that it is a component of FDA-approved drugs, while FDA warned a research-chemical seller in June 2026 that its gonadorelin was an unapproved new drug. WADA bans it in men at all times.

How Gonadorelin works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target GnRH receptor on anterior pituitary gonadotropes (agonist)

Gonadorelin binds the GnRH receptor on pituitary gonadotrope cells and triggers release of LH and FSH. The pituitary responds only to pulses: in monkeys with no hypothalamic signal, hourly pulses restored LH and FSH release while a continuous infusion did not and could shut it down again. That is why fertility pumps deliver a pulse every 90 minutes, and why a single injection is used as a diagnostic test.

  1. 1 Pulse reaches the pituitary Gonadorelin given as a pulse, by pump or injection, reaches GnRH receptors on the pituitary's gonadotrope cells within minutes.
  2. 2 LH and FSH release The gonadotropes release LH and FSH. In the diagnostic test, LH is measured at 15, 30, 45, 60 and 120 minutes after a 100 mcg dose.
  3. 3 Gonads respond LH drives testosterone in the testes and ovulation in the ovaries; FSH drives sperm production and follicle growth.
  4. 4 Fast breakdown The peptide is cut into inactive fragments, mainly by the kidney, with an initial half-life of about 2 to 10 minutes, so each pulse ends quickly.
  5. 5 Continuous exposure backfires If GnRH is present constantly instead of in pulses, the pituitary desensitizes and LH and FSH release falls, as shown in rhesus monkeys in 1978.

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

A Evidence

Gonadorelin is an approved drug with decades of human use. Pulsatile gonadorelin reliably induces ovulation in women with hypothalamic amenorrhea, mostly from open studies, and induces puberty and sperm production in men with congenital hypogonadotropic hypogonadism. There is no published study of gonadorelin injections given with testosterone therapy, which is how it is mostly sold in 2026.

These are questions the published research has not answered.

  1. 01 Whether intermittent gonadorelin injections preserve testicular size or sperm production in men on testosterone therapy: no published trial exists
  2. 02 Whether once- or twice-daily injections, as compounded products are often used, act more like pulses or like a slow shutdown of the pituitary

How long it stays in the body

Half-life
About 4 min
About 2 to 10 minutes initially and 10 to 40 minutes terminal after IV injection; the UK label gives about 4 minutes.
Routes studied
Subcutaneous injection, Intravenous injection, Subcutaneous or IV pulsatile pump, Intramuscular injection (veterinary)
Bioavailability
Subcutaneous relative to intravenous: about 45% at a 5 mcg pulse and 81% at a 20 mcg pulse, per the Canadian Lutrepulse monograph.
Notes
Clearance is high (about 500 to 1,500 L/day) and volume of distribution modest (about 10 to 15 L). The kidney is the main organ of clearance; kidney failure, but not liver disease, prolongs the half-life. Pharmacokinetics look similar in healthy people and in patients with hypothalamic hypogonadism.

Groups with specific risks
  • Known hypersensitivity to gonadorelin or any component
  • Ovarian cysts (Lutrepulse label)
  • Hormone-dependent tumors, since treatment raises sex hormones
  • Conditions that pregnancy could worsen, such as pituitary prolactinoma (Lutrepulse label)
  • Male athletes under WADA rules: prohibited at all times
Interactions
  • Clomiphene: the one moderate hyperstimulation case in Lutrepulse trials occurred in a cycle that also used clomiphene.
  • Testosterone therapy: testosterone feeds back on the pituitary; no published study has measured whether gonadorelin injections overcome that suppression.
  • Long-acting GnRH agonists or antagonists (leuprolide, triptorelin, cetrorelix) act on the same receptor and would blunt or change the response.

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"Gonadorelin is a proven replacement for hCG on testosterone therapy."UntestedNobody has measured this in people

Proven uses involve a pump delivering a pulse every 60 to 120 minutes. No published study has tested daily or twice-daily injections alongside testosterone.

The claim"Gonadorelin is FDA-approved, so compounded versions are approved too."Not supportedThe evidence does not back this

The approved human brands, Factrel and Lutrepulse, are discontinued. Compounded gonadorelin is made under pharmacy rules and is not itself FDA-approved or reviewed.

The claim"More gonadorelin means more testosterone."Not supportedThe evidence does not back this

Constant exposure to GnRH shuts LH and FSH release down. That is how long-acting GnRH agonists suppress testosterone in prostate cancer.

The claim"Pulsatile gonadorelin works well for hypothalamic infertility."SupportedHuman evidence backs this

In 268 women in the Lutrepulse studies, 94% with primary hypothalamic amenorrhea ovulated, and a Swiss cohort reported a 65.9% live birth rate per treatment.

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

Testosterone therapy plus gonadorelin

Some men's health clinics add compounded gonadorelin to testosterone, as a substitute for hCG, to try to keep the testes producing testosterone and sperm.

EvidenceNo published study has tested this combination. Evidence for gonadorelin in men comes from pulsatile pumps in congenital hypogonadotropic hypogonadism, not from daily injections on top of testosterone.

Storage and stability

Dry powder (lyophilized)

The Lutrepulse label states the powder and its saline diluent are stable at room temperature, 15 to 30 C, in the unopened package.

Mixed with water (reconstituted)

Lutrepulse is reconstituted immediately before use and one pump reservoir lasts 72 hours. The label says to discard unused reconstituted solution.

Frequently asked questions
Is gonadorelin FDA approved?
It was. FDA approved Factrel in 1982 and Lutrepulse in 1989, and both human products are now discontinued in the US. It is still FDA-approved for cattle under brands such as Factrel and Cystorelin.
Can pharmacies still compound gonadorelin in 2026?
Compounding pharmacies make it under section 503A, which allows bulk substances that meet a USP monograph or are components of FDA-approved drugs. It is not on any of FDA's 503A category lists and was not part of the July 2026 advisory committee votes. FDA has warned at least one research-chemical seller that its gonadorelin is an unapproved new drug.
Does gonadorelin work like hCG on TRT?
Not in the same way. hCG acts directly on the testes like LH. Gonadorelin acts on the pituitary and only works when given in pulses. No published trial has tested gonadorelin injections for keeping the testes active during testosterone therapy.
What is the half-life of gonadorelin?
Very short. After an IV dose the initial half-life is about 2 to 10 minutes and the terminal half-life 10 to 40 minutes. That is why fertility treatment uses a pump that gives a pulse every 90 minutes.
Is gonadorelin banned in sports?
Yes, for men. WADA's S2.2.1 names gonadorelin among testosterone-stimulating peptides prohibited in males at all times.
What is the difference between gonadorelin and leuprolide?
Gonadorelin is identical to natural GnRH and lasts minutes. Leuprolide is a modified, long-acting GnRH agonist; given continuously it shuts the pituitary down, which is how it lowers testosterone and estrogen.
